Why is image fit causing us fits?

Question: Image fit issues on a press are driving us crazy. We print labels 40 up on a newer 40´´ 6-color litho press. Any ideas?
Answer: Fit issues can drive any pressman crazy fast. Now for a plan of attack:
Setp 1
The first step is to measure plat- to-blanket pressure, then pull break-away dry solids of every unit in cyan ink at a density of 1.40. When you are sure that the blanket-to-impression cylinder is correct then put on a set of PIA/GATF grid plates with color bars. The first pull should be within one line. Now fit the lead edge and the center going around the cylinder.
Cornering the Problem
Evaluate the four corners. What should happen is the lead edge should fit well and the two rear corners will be slightly out. Keep in mind that no press fits perfectly. If the lead edge cannot be fit (i.e. the dots will not stack), you have a serious issue and need to have a press mechanic in. If the first pulls are over one line out, you need to check plate punches and benders.
I'd love to blame the prep department, but if you are CTP then it is hard to blame them.
